Becoming an Authorized Dealer
By clicking on the "Application Forms" button below, you will receive the following documents needed to apply to become a Wijit dealer (in PDF format together as a 107 KB ZIP file):
- Wijit Dealer Application Form
- Sales Tax Exemption Certificate
- Bank Credit Reference Form

To become an authorized dealer of the Wijit, the following requirements apply:
- Registered Durable Medical Equipment Dealer.
- Proof of a valid Resale License for your region (state).
- Current Federal I.D. (FEIN).
As an authorized dealer, SuperQuad (producers of the Wijit) will provide you with:
- Wijit Authorized Dealer package that includes brochures and promotional materials.
- Product technical support.
- Copies of local and national news releases about the Wijit.
- Listing on the SuperQuad web site Dealer Locator.
- Leads for your area as they are generated by SuperQuad's web site and publicity efforts.
The Wijit Dealer Packet contains information specific to dealers, such as pricing, suggested retail, obtaining demo units, and more.

Upon completion of the form submission, you will be taken to a page where you can download the Dealer Packet and/or receive it by mail if the "Mail" checkbox is selected.
